The Assistant Director of Rehabilitation Services is responsible for providing high-quality therapy services that improve the lives of residents. In this position, you will provide therapy treatments and ensure that residents always receive exemplary care. During absences, this individual assumes the duties of the Director of Rehabilitation Services.
  • Skilled in therapy treatments and rehabilitation goals; able to maintain required contact with physician.
  • Keeps written evaluations, progress notes, and discharge summaries of patients in therapy programs that follow state, federal, and facility policies; suggests written inter-disciplinary care plans and treatments.
  • Translates and adapts goals from other team members to create an overall rehabilitation care plan.
  • Provides input for Resident Care Planning (RCP) and Resident/Family Care Plan Review meetings; attends when appropriate; completes required documentation after the meetings.
  • Maintains statistical records of residents’ treatments.
  • Assists with education and in-service training when needed.
  • Records and updates resident's care plan(s).
  • Keeps residents or their qualified representatives informed about care and treatment plans, possible outcomes of therapy programs, or what to expect if a resident refuses participation.
  • Trains and supervises department staff while also helping students with facility or educational program concerns.
  • This position is responsible for submitting requisitions for supplies and equipment.
  • Acts as the Therapy/Rehab direct report in tandem with the Director.
  • This position is responsible for ensuring that all documentation is processed accurately and in a timely manner.
  • In the absence of a Director, Leads departmental meetings (when necessary) and participates in the following inter-departmental gatherings as required:

  1. Resident Care Planning
  2. Quality Assurance Committee
  3. Quality Assessment and Review Committee
  4. Morning Report
  5. Safety Committee
  6. In-service Education
  7. Staff Meeting for Department heads and Supervisors

Who Will Be a Great Fit?  
  • State of NY Physical Therapist License in good standing
  • MS in Physical Therapy or Occupational Therapy
  • Previous management experience in a Skilled Nursing setting
  • The ideal candidate will have a passion for working with the elderly and an innate understanding of their rights during care services.
  • Genuine concern for the elderly with a caring demeanor
