Key Responsibilities:

• Initiate skilled occupational therapy treatments based on physician's orders and maintain ongoing communication with the medical team.
• Create and maintain written evaluations, progress notes, MDS records, and discharge summaries for residents in the occupational therapy program.
• Collaborate with healthcare colleagues to address the unique needs of residents, enhancing their rehabilitation and function maintenance.
• Participate in Resident Care Planning meetings and cover in the absence of the Director of Occupational Therapy.
• Maintain accurate statistical records of residents' treatments and progress.
• Assist in delivering in-service training programs when requested.
• Record and update residents' care plans to ensure comprehensive and high-quality therapy services.

• Graduated from an accredited school of Occupational Therapy.
• Holds a valid New York State occupational therapy license in good standing.
• Prior experience in a skilled nursing facility (SNF) is preferred.
• Committed to providing comprehensive, quality therapy services to promote residents' health and well-being.
• Capable of administering skilled occupational therapy treatments while upholding the highest standards of resident care.
