Key Responsibilities:
As a valued member of our physical therapy team, your role is dedicated to delivering high-quality services that enhance the health and well-being of our residents. You will skillfully administer physical therapy treatments in accordance with physician directives and maintain essential communication with physicians. The physical therapist is responsible for composing and maintaining evaluations, progress notes, and discharge summaries for residents participating in the physical therapy program. Collaborating with fellow healthcare professionals, you will help develop specialized plans to improve a resident's functional abilities or maintain their current level of functioning. Your active participation in Resident Care Planning meetings and attendance during the Director of Physical Therapy's absences are vital. Additionally, you will maintain meticulous statistical records of residents' treatments.

Who Will Be a Great Fit? 

• Master's or Doctorate in Physical Therapy (MS/DPT)
• A valid State of New York Physical Therapy license in good standing
• Prior experience as a Physical Therapist in a Skilled Nursing Facility
