• The physical therapy staff are dedicated to providing residents with high-quality services that promote health and well-being.
• Skillfully administers physical therapy treatments as dictated by physician; keeps required contact with physician.
• The physical therapist writes and maintains evaluations, progress notes, and discharge summaries for residents in the physical therapy program.
• Consulting with other health care staff to develop special plans that will improve a resident's ability to function or help them maintain their current level of functioning.
• Provides input for the Resident Care Planning meetings and attends during the Director of Physical Therapy absences.
• Maintains statistical records of residents’ treatments.

Who Will Be a Great Fit?  

• MS/DPT in Physical Therapy 
• Must have a State of New York Physical Therapy license in good standing
• Experience as a Physical Therapist in a Skilled Nursing Facility
