    Who Will Be a Great Fit?

  • Educational Requirement: Possession of an MS/DPT in Physical Therapy.
  • Licensure: Must hold a valid State of New York Physical Therapy License in good standing.
  • Experience: Previous experience in a Skilled Nursing Facility is preferred but not mandatory. New graduates are encouraged to apply.
  • Skills: You have a passion for providing care that improves your patients' quality of life, excellent communication skills, and the ability to work effectively within a multidisciplinary team.
  • Key Responsibilities:

  • Deliver High-Quality Physical Therapy Services: Dedicate yourself to providing the best physical health and well-being services to our residents.
  • Administer Treatments and Keep Records: Skillfully carry out physical therapy treatments as prescribed and maintain up-to-date records, including evaluations, progress notes, and discharge summaries.
  • Collaborate with Healthcare Staff: Work closely with other healthcare professionals to design and implement customized care plans.
  • Contribute to Resident Care Planning: Offer valuable insights during Resident Care Planning meetings and represent the department in the absence of the Director of Physical Therapy.
  • Maintain Accurate Records: Keep precise statistical records of treatments to ensure each resident's progress is accurately tracked.
