Key Responsibilities:

• Assessment & Treatment Planning:
  • Conduct comprehensive evaluations of residents' needs based on medical diagnoses.
  • Develop individualized treatment plans aimed at enhancing residents' functional independence.
• Implementation & Documentation:
  • Implement therapeutic interventions to achieve treatment goals.
  • Document residents' progress meticulously through detailed re-certifications, progress notes, and daily entries.
• Clinical Supervision & Collaboration:
  • Provide clinical supervision to occupational therapy assistants.
  • Actively participate in interdisciplinary care plan meetings.
• Quality Assurance & Reporting:
  • Ensure confidentiality and integrity of resident information.
  • Audit resident and department records to maintain high-quality care services.
  • Contribute to OT reports and data tracking as instructed by the Rehabilitation Director.
• Project Participation:
  • Assist with departmental projects and initiatives as required.

Requirements:

• Educational Background:
  • Must be a graduate of an accredited Occupational Therapy Program.
• Licensure:
  • Current New York State OT License is required.
• Experience:
  • Prior experience is beneficial but not mandatory.
