Physical Therapist (Float between Outpatient and Inpatient)

Join our Premier Health Clinic's Physical Therapy team and make a significant impact on patients' lives. We are seeking a dedicated Physical Therapist to float between our outpatient and inpatient facilities across Bath, Corning, Horseheads, and Elmira, NY. This full-time, day shift position offers a competitive salary based on experience, an excellent benefits package, relocation assistance, and a $5000 sign-on bonus.

Pay Rate =  $71,000 - $100000
 

Key Responsibilities:

  • Treatment Planning and Implementation:
    • Develop and implement individualized treatment programs based on physical therapy principles.
    • Collaborate with interdisciplinary treatment teams.
    • Evaluate patients' physical conditions and needs through interviews and assessments.
    • Determine appropriate equipment usage and adjust treatments as necessary.
    • Instruct patients on the proper use of mobility aids and prosthetic devices.
    • Modify treatment programs based on patient progress and consult with physicians as needed.
 
  • Documentation and Communication:
    • Prepare required documentation such as evaluation results, treatment plans, progress reports, and discharge summaries.
    • Communicate effectively with nursing staff and interdisciplinary team members to ensure coordinated patient care.
 
  • Guidance and Supervision:
    • Provide direction to staff members assisting in patient care.
    • Maintain department policies, procedures, objectives, quality initiatives, safety, and infection control standards.
 
  • Safety and Compliance:
    • Adhere to hospital-wide safety responsibilities and protocols.
    • Ensure proper use of safety devices and report any unsafe conditions.
    • Participate in safety programs and maintain knowledge of hospital safety policies.
 
  • Professional Development:
    • Enhance professional growth through educational programs, literature review, in-service meetings, and workshops.
    • Attend all mandatory educational programs required by the position.
 
  • Patient Care:
    • Provide age-appropriate care to neonate, infant, children, adolescent, adult, and geriatric patients.
    • Ensure a friendly and caring interaction with patients to meet their psychological and physical needs.
 

Qualifications:

  • Must be licensed or eligible to obtain a license to practice physical therapy in NYS at the time of hire.
  • Experience in a hospital setting is preferred.
  • Basic Life Support (BLS) certification required within 90 days of hire.
