• Conduct Start of Care (SOC) assessments and evaluations
• Perform regular follow-up visits and patient monitoring
• Manage discharge and re-certification processes
• Provide wound care and medication management
• Administer medications and intravenous therapies

• Collaborate with physicians and make care recommendations
• Maintain detailed and accurate patient records
• Active, unrestricted California Registered Nurse (RN) license
• Minimum 1 year of professional nursing experience in Home Health or Acute Care
• Valid California Driver's License with a clean DMV record
• Excellent communication and interpersonal skills
• Proficiency in electronic health record systems
