Join our dynamic team as a Physical Therapist in our Skilled Nursing Facility, where you'll make a significant impact on residents' lives. We're seeking a compassionate and skilled PT to develop and administer therapy plans, promote mobility, and enhance overall quality of life for our residents. New graduates are welcome, and experience in long-term care is a plus!

 

Key Responsibilities

• Conduct thorough evaluations of residents' conditions upon admission
• Develop and implement personalized physical rehabilitation plans
• Utilize exercise therapy, manual therapy, and other techniques to improve mobility and reduce pain
• Educate residents on proper use of physical therapy equipment
• Monitor and document resident progress, adjusting treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive resident care
• Supervise physical therapy assistants when required
• Maintain a safe, clean, and confidential working environment
• Adhere to infection-control policies and protocols
• Comply with federal, state, and local regulations and professional requirements

 

Job Requirements

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Commitment to ongoing professional development
• Experience in skilled nursing or long-term care (preferred but not required)
