Clinical Pharmacy Coordinator - Hospital Setting (PGY-2 Preferred)

A leading healthcare institution in Texas is seeking an experienced Clinical Pharmacy Coordinator to join our dynamic pharmacy team. This role combines clinical expertise with leadership responsibilities to optimize patient care and advance our pharmaceutical services.

Essential Responsibilities:

  • Lead clinical pharmacy initiatives and antimicrobial stewardship programs
  • Develop and implement evidence-based clinical protocols
  • Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
  • Provide clinical guidance and mentorship to pharmacy staff
  • Participate in medication safety and quality improvement initiatives
  • Oversee pharmacy residency programs and student education
  • Monitor and evaluate clinical pharmacy services effectiveness

Required Qualifications:

  • Doctor of Pharmacy (Pharm.D.) from an accredited institution
  • Active Texas State Board of Pharmacy license
  • PGY-1 residency completion plus 2 years clinical experience OR PGY-2 residency completion
  • ACPE IV certification
  • Current ACLS and AHA BLS certifications
  • Demonstrated experience in acute care setting

Preferred Qualifications:

  • PGY-2 residency completion
  • Board of Pharmacy Specialties (BPS) certification
  • Antimicrobial stewardship certification
  • Leadership experience in hospital pharmacy setting
