Senior Physical Therapist - Home Health Care
We're seeking a dedicated Senior Physical Therapist to join our patient-centered home health care team in delivering exceptional therapeutic services. - Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
- Deliver evidence-based therapeutic interventions in patients' homes
- Create and implement custom rehabilitation programs focusing on functional improvement
- Provide detailed education to patients and caregivers on exercises and therapeutic techniques
- Maintain accurate documentation and progress reports using electronic health records
- Collaborate with physicians and healthcare team members to optimize patient outcomes
- Manage and prioritize patient caseload efficiently
Required Qualifications:
- Current Physical Therapy license in Arizona
- Bachelor's or Master's degree in Physical Therapy (DPT preferred)
- Minimum 2 years of clinical experience
- Valid driver's license and reliable transportation
- BCLS certification (obtainable within 30 days of hire)
- Strong interpersonal and communication skills
