Clinical Services Manager - Home Health (RN/PT/OT)

We're seeking an experienced Clinical Services Manager to lead our Home Health team in delivering exceptional patient care. This role combines clinical expertise with leadership responsibilities, overseeing quality care delivery while managing team performance and operational excellence.

Essential Responsibilities:

• Lead and supervise clinical team members, providing ongoing coaching and performance management
• Oversee quality of care and program compliance with federal, state, and CHAP regulations
• Coordinate comprehensive patient care services, including care planning and referral management
• Analyze clinical outcomes data and implement quality improvement initiatives
• Manage departmental budget and resource allocation
• Monitor and enhance patient satisfaction metrics
• Investigate and resolve patient concerns
• Ensure documentation accuracy and compliance with reimbursement criteria

Required Qualifications:

• Current licensure as a Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT)
• Bachelor's degree in related healthcare field
• Minimum 3 years clinical experience in nursing/patient care
• Current CPR certification
• Valid driver's license and auto insurance
• Strong analytical and problem-solving abilities
• Excellent communication and leadership skills

Preferred Qualifications:

• Master's degree in healthcare administration or related field
• 2+ years of supervisory experience in healthcare setting
• Home Health care experience
• Knowledge of CHAP standards and Medicare regulations
