Clinical Pharmacy Coordinator - Antimicrobial Stewardship Specialist

A leading healthcare institution in Texas is seeking an experienced Clinical Pharmacy Coordinator to join our dynamic pharmacy team. This role combines clinical expertise with leadership responsibilities, focusing on advancing patient care through evidence-based pharmacy practices and antimicrobial stewardship.

Essential Responsibilities:

• Lead clinical pharmacy initiatives and medication safety programs
• Oversee antimicrobial stewardship efforts and disease state management protocols
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• Provide clinical consultation and drug therapy recommendations
• Develop and implement clinical pharmacy guidelines and protocols
• Mentor pharmacy residents and staff
• Participate in quality improvement initiatives

Required Qualifications:

• Doctor of Pharmacy (PharmD) from an accredited institution
• Active Texas State Board of Pharmacy license
• PGY-1 residency completion plus 2 years clinical experience OR PGY-2 residency completion
• ACPE IV certification
• ACLS and AHA BLS certifications
• Hospital pharmacy residency with specialty training in Disease State Management

Preferred Qualifications:

• Antimicrobial stewardship certification
• Board of Pharmacy Specialties (BPS) certifications
• Two years of residency training
