Director of Clinical Case Management - Hospital Setting

A leading healthcare facility is seeking an experienced Director of Clinical Case Management to lead our comprehensive care coordination and discharge planning initiatives. Position Overview

As the Director of Clinical Case Management, you will lead a dynamic team while overseeing all aspects of hospital case management operations. This role combines clinical expertise with executive leadership to optimize patient care transitions, resource utilization, and departmental performance.

Essential Responsibilities

  • Lead and optimize facility-wide case management operations, including staffing, workflow design, and performance metrics
  • Develop and implement strategic initiatives to enhance care coordination and discharge planning processes
  • Drive compliance with regulatory requirements and organizational policies while maintaining quality standards
  • Foster collaborative relationships with key stakeholders including CMO, CNO, and departmental leaders
  • Monitor and analyze operational metrics to identify opportunities for process improvement
  • Manage resource allocation and budget to achieve organizational objectives
  • Ensure best practices in care transitions and length-of-stay management
  • Lead performance improvement initiatives and staff development programs

Required Qualifications

  • Current RN license in good standing
  • Bachelor's degree in Nursing or related field
  • Minimum 2 years of recent (within last 2 years) experience as Director of Case Management in a large acute care setting
  • Proven track record of leadership success in healthcare operations
  • Strong analytical and decision-making capabilities
  • Excellence in interpersonal communication and team building

Preferred Qualifications

  • Master's degree in Nursing, Healthcare Administration, or Business Administration
  • Case Management Certification (ACM or CCM)
  • Experience with healthcare information systems and analytics
