Director of Clinical Case Management & Care Coordination

We are seeking an experienced Clinical Case Management Director to lead and transform our case management department at our acute care facility. This executive leadership position offers the opportunity to drive excellence in patient care coordination, optimize clinical resource utilization, and lead a dedicated team of healthcare professionals.

Position Overview

As Director of Clinical Case Management, you will oversee all operational aspects of the department while ensuring optimal patient outcomes, regulatory compliance, and departmental performance. This role combines clinical expertise with strategic leadership to enhance care coordination across the healthcare continuum.

Essential Responsibilities

  • Lead and develop a high-performing case management team while optimizing staffing models and productivity metrics
  • Drive clinical excellence through implementation of evidence-based case management practices and care coordination protocols
  • Oversee utilization management processes to ensure appropriate resource allocation and length of stay management
  • Collaborate with executive leadership to achieve strategic objectives and financial targets
  • Develop and maintain relationships with key stakeholders including physicians, payers, and post-acute providers
  • Ensure regulatory compliance and documentation integrity across case management functions
  • Monitor and analyze department performance metrics to drive continuous improvement initiatives
  • Lead change management efforts to enhance departmental efficiency and effectiveness

Required Qualifications

  • Current, unrestricted RN license
  • Bachelor's degree in Nursing or related field
  • Minimum 2 years of recent (within past 2 years) director-level case management experience in an acute care hospital setting
  • Demonstrated success in leading clinical operations and achieving organizational objectives
  • Strong understanding of healthcare regulations, reimbursement models, and quality metrics
  • Excellence in communication, presentation, and interpersonal skills

Preferred Qualifications

  • Master's degree in Nursing, Healthcare Administration, or Business Administration
  • Case Management Certification (ACM, CCM)
  • Experience with performance improvement methodologies
