What You’ll Do:

• Administer effective physical therapy treatments according to physician’s orders, ensuring optimal outcomes for residents.
• Evaluate residents’ conditions, create customized care plans, and maintain detailed progress notes, evaluation summaries, and discharge summaries.
• Collaborate with other healthcare team members to develop strategies that promote independence or maintain current levels of functioning.
• Participate in Resident Care Planning meetings, offering valuable input on resident improvement plans or strategies.
• Step in to represent the Physical Therapy Department at care meetings during the Director of Physical Therapy's absence.
• Maintain accurate statistical records of residents’ treatments for internal and regulatory purposes.

Who Will Be a Great Fit:

• Master’s or Doctorate degree in Physical Therapy (MS/DPT).
• Active and valid New York State Physical Therapy license in good standing.
• At least one year of experience as a Physical Therapist in a Skilled Nursing Facility is preferred.
