Clinical Operations Manager - Home Health Services

A leading home healthcare organization seeks a dynamic Clinical Operations Manager to drive excellence in patient care delivery and operational efficiency. This role offers an exciting opportunity to lead clinical operations while ensuring the highest standards of healthcare delivery in a rapidly growing home health environment.

Position Overview

Full-time position (8am - 5pm) focused on overseeing clinical operations, quality assurance, and regulatory compliance, while supporting our clinical team in delivering exceptional patient care.

Key Responsibilities

  • Lead day-to-day clinical operations and ensure compliance with federal, state, and agency regulations
  • Coordinate patient referrals and oversee care delivery through an interdisciplinary team approach
  • Manage OASIS documentation, including discharge assessments and data analysis
  • Drive QAPI initiatives and maintain survey readiness standards
  • Supervise clinical competency assessments and ongoing professional development
  • Monitor key performance indicators (KPIs) and implement quality improvement measures
  • Oversee emergency preparedness planning and response protocols
  • Manage HHCAHPS reporting and analysis through SHP

Required Qualifications

  • Bachelor's degree in nursing, healthcare administration, or related field (Master's preferred)
  • Active nursing license or relevant clinical licensure
  • Minimum 3 years of home health leadership experience
  • Demonstrated expertise in OASIS documentation and compliance
  • Strong background in QAPI processes and survey preparation
  • Proficiency in EHR systems and healthcare technology platforms
  • Exceptional analytical, leadership, and communication skills
