Position Overview

As a Home Health Physical Therapist, you'll provide expert therapeutic care while enjoying the autonomy and flexibility of managing your own patient caseload. This role offers competitive compensation ($105,000-$150,000 annually) and the opportunity to build lasting relationships with patients and their families.

Essential Responsibilities

• Perform comprehensive physical therapy evaluations and functional assessments
• Develop and implement evidence-based treatment plans aligned with patient goals
• Deliver hands-on therapeutic interventions and exercise programs
• Provide expert guidance on mobility, safety, and injury prevention
• Document patient progress and maintain detailed electronic health records
• Coordinate care plans with physicians, nurses, and other healthcare providers
• Demonstrate excellent time management while traveling between patient homes

Required Qualifications

• Master's degree in Physical Therapy from an accredited program
• Current Physical Therapy license in California
• Minimum 1 year of home health PT experience
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong interpersonal and communication skills
• Proficiency in electronic medical record systems
