Licensed Physical Therapist Assistant (PTA) - Home Health Care
We're seeking a compassionate and skilled Physical Therapist Assistant (PTA) to join our growing home health care team. In this role, you'll work directly with patients in their homes, implementing personalized treatment plans and helping them achieve their mobility goals while maintaining the highest standards of patient care.
Core Responsibilities:
- Deliver hands-on therapeutic interventions and exercise programs to home health patients
- Implement treatment plans under the supervision of licensed Physical Therapists
- Provide expert instruction in therapeutic exercises, mobility techniques, and use of assistive devices
- Document patient progress and maintain detailed clinical records in compliance with healthcare regulations
- Educate patients and caregivers on proper exercise techniques and home exercise programs
- Assess patient progress and communicate findings to the healthcare team
- Travel to multiple patient homes within the assigned territory
Required Qualifications:
- Associate's Degree from an accredited PTA program
- Current state PTA license in good standing
- Valid CPR certification
- 1+ year of home health care experience
- Valid driver's license and reliable transportation
- Strong interpersonal and communication skills
- Physical ability to assist patients with mobility and exercises
