Position Overview

As a Home Health Physical Therapist, you'll deliver expert rehabilitation services while working autonomously within a supportive multidisciplinary team. This role offers flexible scheduling and the chance to build lasting relationships with patients throughout their recovery journey.

Essential Responsibilities

• Perform comprehensive physical therapy evaluations and functional assessments
• Develop and implement evidence-based treatment plans
• Provide therapeutic interventions including manual therapy, exercise prescription, and gait training
• Deliver patient/caregiver education on self-management techniques
• Document care using EMR systems according to Medicare guidelines
• Coordinate care plans with physicians, nurses, and other healthcare providers
• Participate in quality improvement initiatives

Required Qualifications

• Master's or Doctorate in Physical Therapy from an accredited program
• Current California Physical Therapy license
• Minimum 1 year home health experience
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong communication and interpersonal skills
• Proficiency in EMR documentation
