Senior Physical Therapist Assistant - Home Health Care Specialist

• Maintain detailed patient documentation in compliance with healthcare regulations
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
• Monitor and evaluate patient progress, adjusting treatment approaches as needed
• Promote wellness and provide patient education for long-term health maintenance

Qualifications & Requirements:

• Master's degree in Physical Therapy required
• Current state Physical Therapist license
• Minimum 1 year of home health care experience
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong interpersonal and communication skills
• Proven experience with orthopedic and neurological rehabilitation
