Position Overview

As a Transplant Clinical Pharmacist, you'll manage the complete spectrum of pharmaceutical care for transplant patients, from initial evaluation through long-term follow-up care. This role combines clinical expertise with patient advocacy to ensure optimal outcomes in transplant medicine.

Key Responsibilities

• Provide comprehensive medication management throughout the transplant continuum of care
• Participate in multidisciplinary patient selection committees
• Conduct pre-transplant medication evaluations and patient counseling
• Manage complex immunosuppressive medication regimens
• Lead medication education for patients and families
• Develop and implement treatment protocols for allograft rejection
• Precept pharmacy students and residents
• Contribute to clinical research and quality improvement initiatives

Required Qualifications

• PharmD from an ACPE-accredited College of Pharmacy
• Current Virginia pharmacist license or eligibility for licensure
• Completion of ASHP-accredited PGY1 Pharmacy Residency
• Strong clinical decision-making and analytical skills
• Excellence in interprofessional communication
• Demonstrated leadership and project management abilities

Preferred Qualifications

• PGY2 Solid Organ Transplant Pharmacy Residency
• Clinical research experience
• Previous transplant center experience
