Per Diem Registered Nurse (RN) - Home Health Care Specialist

Core Responsibilities:
- Deliver expert clinical care to diverse patient populations in their homes
- Perform comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments according to physician orders
- Document patient care using electronic health records (EHR)

- Provide patient and family education on health management
- Monitor vital signs and track patient progress
- Ensure compliance with healthcare regulations and standards
Required Qualifications:
- Current California RN license in good standing
- Minimum 6 months home health care experience
- One year recent acute care experience
- Valid driver's license and insured vehicle
- Current CPR certification
- Graduate of accredited nursing program (BSN preferred)
- Public Health Nurse (PHN) certification preferred
