Home Health Clinical Manager (RN)

Key Responsibilities

  • Lead Clinical Teams: Supervise and support nurses, therapists, and home health aides—including hiring, onboarding, scheduling, mentoring, and performance evaluations.

  • Coordinate Patient Care: Oversee individualized care plans in collaboration with physicians and allied health professionals to ensure timely and effective interventions.

  • Ensure Compliance: Maintain strict adherence to federal, state, and local healthcare regulations, including Medicare/Medicaid guidelines and Joint Commission standards.

  • Drive Quality Assurance: Conduct audits, track clinical outcomes, and spearhead continuous improvement initiatives for optimal patient results.

  • Advocate for Patients: Serve as a liaison to address concerns, resolve issues promptly, and enhance overall patient and family satisfaction.

  • Manage Documentation: Review and approve clinical notes and assessments for accuracy, completeness, and regulatory alignment.

  • Assist with Budget Oversight: Help manage departmental budgets, focusing on resource optimization without compromising care quality.

  • Educate & Train: Provide ongoing training to clinical staff on best practices, policy updates, and compliance standards.

  • Foster Collaboration: Work closely with administrative leaders, case managers, and external partners to streamline workflows and elevate care outcomes.


Required Qualifications

  • Education: Bachelor of Science in Nursing (BSN) required; Master’s in Nursing or Healthcare Administration preferred.

  • Licensure: Active RN license in the state of Arizona.

  • Experience: 2–3+ years in home health or a related field, with prior supervisory or management experience.

  • Certifications: BLS certification required; Home Health Care Certification preferred.

  • Technical Skills: Proficient in Electronic Health Records (EHR) systems and clinical software.

  • Soft Skills: Strong leadership, excellent communication, decision-making, and team-building abilities.

  • Regulatory Knowledge: Deep understanding of Medicare/Medicaid reimbursement and home health compliance.
