Transplant Clinical Pharmacist - Solid Organ Specialist

Core Responsibilities:
- Provide direct patient care throughout the transplant journey
- Participate in multidisciplinary patient selection committees
- Conduct pre-operative clinic visits and patient counseling
- Manage inpatient medication therapy during acute transplantation
- Optimize immunosuppressive regimens and monitor therapeutic outcomes
- Deliver comprehensive medication education to patients and families
- Develop and implement rejection treatment protocols
- Precept pharmacy students and residents
- Lead research initiatives and quality improvement projects
Required Qualifications:
- PharmD from ACPE-accredited College of Pharmacy
- Virginia pharmacist license or eligible for licensure within 90 days
- Completed ASHP-accredited PGY1 Pharmacy Residency
- Strong clinical decision-making and analytical skills
- Excellence in leadership and interpersonal communication
- Demonstrated ability to work independently and collaboratively
Preferred Qualifications:
- PGY2 Residency in Solid Organ Transplantation
- Previous transplant pharmacy experience
- Research experience in transplant medicine
