Transplant Clinical Pharmacist - Solid Organ Specialist

Join our prestigious healthcare team as a Transplant Clinical Pharmacist specializing in solid organ transplantation. This role offers an exceptional opportunity to provide comprehensive pharmaceutical care across the entire transplant continuum while working with a distinguished multidisciplinary team in our state-of-the-art facility.

Core Responsibilities:

  • Provide direct patient care throughout the transplant journey
  • Participate in multidisciplinary patient selection committees
  • Conduct pre-operative clinic visits and patient counseling
  • Manage inpatient medication therapy during acute transplantation
  • Optimize immunosuppressive regimens and monitor therapeutic outcomes
  • Deliver comprehensive medication education to patients and families
  • Develop and implement rejection treatment protocols
  • Precept pharmacy students and residents
  • Lead research initiatives and quality improvement projects

Required Qualifications:

  • PharmD from ACPE-accredited College of Pharmacy
  • Virginia pharmacist license or eligible for licensure within 90 days
  • Completed ASHP-accredited PGY1 Pharmacy Residency
  • Strong clinical decision-making and analytical skills
  • Excellence in leadership and interpersonal communication
  • Demonstrated ability to work independently and collaboratively

Preferred Qualifications:

  • PGY2 Residency in Solid Organ Transplantation
  • Previous transplant pharmacy experience
  • Research experience in transplant medicine
