Key Responsibilities:

• Collaborate with interdisciplinary care teams to develop and implement optimal treatment plans
• Provide expert pharmacotherapy consultations and medication therapy management
• Participate in clinical decision-making and evidence-based protocol development
• Offer virtual pharmacy consultations for ambulatory care settings
• Deliver comprehensive medication education to patients and healthcare providers
• Review and verify medication orders for safety and appropriacy
• Oversee sterile/non-sterile compounding operations
• Respond to medical emergencies as part of the code team
• Mentor pharmacy students and residents
• Maintain compliance with regulatory requirements and institutional policies

Required Qualifications:

• PharmD degree from an ACPE-accredited College of Pharmacy
• Virginia State Pharmacy License (or ability to obtain within 90 days of hire)
• Strong clinical knowledge and excellent communication skills
• Commitment to patient safety and quality improvement

Preferred Qualifications:

• PGY1 Pharmacy Residency completion
• Hospital pharmacy experience
• Experience with electronic health record systems
