Key Responsibilities

Conduct comprehensive physical therapy evaluations and assessments for home care and inpatient populations
Develop and implement individualized treatment plans based on patient needs and therapeutic goals
Administer physical therapy interventions including therapeutic exercise, gait training, manual therapy, and modalities (ultrasound, electrical stimulation, hot/cold therapy, traction)
Educate patients and families on proper use of adaptive equipment including wheelchairs, walkers, crutches, canes, braces, and prosthetic devices
Document patient evaluations, treatment plans, progress notes, and discharge summaries in accordance with professional and regulatory standards
Collaborate with interdisciplinary healthcare team members including physicians, nurses, occupational therapists, and case managers
Monitor patient progress and modify treatment plans based on clinical outcomes and physician recommendations
Provide supervision and guidance to physical therapy assistants and support staff
Maintain compliance with department policies, infection control protocols, and safety procedures
Participate in quality improvement initiatives and departmental meetings
Ensure proper maintenance and care of rehabilitation equipment and supplies
Provide age-appropriate care across the lifespan from neonates to geriatric patients
Transport patients to and from therapy sessions as needed
Participate in continuing education and professional development activities

Required Qualifications

Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
Current New York State Physical Therapy license or eligibility to obtain licensure at time of hire
Basic Life Support (BLS) certification required within 90 days of hire
Ability to lift at least 30 pounds independently and 150 pounds with assistance
Strong clinical assessment and documentation skills
Excellent interpersonal and communication skills for patient care and team collaboration
Knowledge of infection control and universal precautions protocols
Commitment to providing patient-centered care with compassion and professionalism

Preferred Qualifications

Previous experience in hospital, inpatient, or home health physical therapy settings
Experience with diverse patient populations across multiple age groups
Familiarity with electronic medical records (EMR) systems
