Physical Therapist - Home Care & Inpatient - Elmira, NY | Up to $15K Sign-On Bonus

Key Responsibilities

  • Conduct comprehensive physical therapy evaluations and assessments for home care and inpatient populations
  • Develop and implement individualized treatment plans based on patient needs and therapeutic goals
  • Administer physical therapy interventions including therapeutic exercise, gait training, manual therapy, and modalities (ultrasound, electrical stimulation, hot/cold therapy, traction)
  • Educate patients and families on proper use of adaptive equipment including wheelchairs, walkers, crutches, canes, braces, and prosthetic devices
  • Document patient evaluations, treatment plans, progress notes, and discharge summaries in accordance with professional and regulatory standards
  • Collaborate with interdisciplinary healthcare team members including physicians, nurses, occupational therapists, and case managers
  • Monitor patient progress and modify treatment plans based on clinical outcomes and physician recommendations
  • Provide supervision and guidance to physical therapy assistants and support staff
  • Maintain compliance with department policies, infection control protocols, and safety procedures
  • Participate in quality improvement initiatives and departmental meetings
  • Ensure proper maintenance and care of rehabilitation equipment and supplies
  • Provide age-appropriate care across the lifespan from neonates to geriatric patients
  • Transport patients to and from therapy sessions as needed
  • Participate in continuing education and professional development activities

Required Qualifications

  • Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
  • Current New York State Physical Therapy license or eligibility to obtain licensure at time of hire
  • Basic Life Support (BLS) certification required within 90 days of hire
  • Ability to lift at least 30 pounds independently and 150 pounds with assistance
  • Strong clinical assessment and documentation skills
  • Excellent interpersonal and communication skills for patient care and team collaboration
  • Knowledge of infection control and universal precautions protocols
  • Commitment to providing patient-centered care with compassion and professionalism

Preferred Qualifications

  • Previous experience in hospital, inpatient, or home health physical therapy settings
  • Experience with diverse patient populations across multiple age groups
  • Familiarity with electronic medical records (EMR) systems
