Physical Therapist - Home Health | Franklin County, NY | Full-Time/Part-Time

Key Responsibilities
- Conduct comprehensive patient evaluations using diagnostic muscle, nerve, joint, and functional ability tests and measures in home settings
- Develop individualized treatment plans and goals based on physician orders and patient assessment findings
- Provide skilled physical therapy interventions to address impairments and functional limitations, maximizing patient independence
- Document clinical notes, assessments, and progress updates in medical records on the date of service
- Educate patients and caregivers on therapeutic exercises, treatment plans, and proper use of assistive devices (walkers, canes, wheelchairs, orthotics)
- Collaborate with physicians to recommend modifications to the plan of care as patient needs evolve
- Perform periodic re-evaluations to measure progress and adjust treatment approaches
- Participate in interdisciplinary case conferences, staff meetings, and continuing education opportunities
- Ensure proper safety practices and maintenance of rehabilitation equipment
- Travel to patients' homes throughout Franklin County to deliver quality physical therapy services
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or MPT)
- Current, valid Physical Therapy license issued by the New York State Department of Education
- Minimum one year of practical experience as a licensed Physical Therapist
- Strong clinical assessment and documentation skills
- Excellent communication and patient education abilities
- Valid driver's license and reliable transportation for home visits
- Ability to lift and transfer patients (up to 50 pounds occasionally)
- Physical capability to perform essential functions including standing, walking, stooping, kneeling, and repetitive motions
