Key Responsibilities

Conduct comprehensive physical therapy evaluations and assessments for skilled nursing facility residents
Develop and implement personalized treatment plans to improve mobility, strength, balance, and overall quality of life
Provide therapeutic interventions including manual therapy, therapeutic exercise, gait training, and functional mobility training
Utilize assistive devices and adaptive equipment to enhance patient independence
Deliver pain management interventions and post-surgical rehabilitation services
Educate patients and families on therapy goals, progress, home exercise programs, and recovery expectations
Monitor patient outcomes and adjust rehabilitation care plans based on clinical progress
Supervise and mentor physical therapy assistants and support staff as appropriate
Maintain accurate and timely clinical documentation in compliance with state and federal regulations
Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure holistic, patient-centered care
Adhere to all safety protocols, infection control standards, and HIPAA regulations

Qualifications & Requirements

Doctor of Physical Therapy (DPT) degree or Master's degree from an accredited physical therapy program
Current and unrestricted New York State Physical Therapy License (required)
CPR/BLS Certification (required or must obtain upon hire)
Previous experience in skilled nursing facility (SNF), long-term care (LTC), or geriatric rehabilitation settings strongly preferred
Knowledge of Medicare guidelines and documentation requirements preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills with a patient-focused approach
Empathy, compassion, and collaborative team spirit
Proficiency with electronic medical records (EMR) systems preferred

Compensation & Benefits

Competitive annual salary: $104,000 - $124,000 based on experience and credentials
