Physical Therapist – Full-Time Direct Hire | Skilled Nursing & Rehabilitation | Poughkeepsie, NY

Key Responsibilities

  • Conduct comprehensive physical therapy evaluations and assessments for skilled nursing facility residents
  • Develop and implement personalized treatment plans to improve mobility, strength, balance, and overall quality of life
  • Provide therapeutic interventions including manual therapy, therapeutic exercise, gait training, and functional mobility training
  • Utilize assistive devices and adaptive equipment to enhance patient independence
  • Deliver pain management interventions and post-surgical rehabilitation services
  • Educate patients and families on therapy goals, progress, home exercise programs, and recovery expectations
  • Monitor patient outcomes and adjust rehabilitation care plans based on clinical progress
  • Supervise and mentor physical therapy assistants and support staff as appropriate
  • Maintain accurate and timely clinical documentation in compliance with state and federal regulations
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure holistic, patient-centered care
  • Adhere to all safety protocols, infection control standards, and HIPAA regulations

Qualifications & Requirements

  • Doctor of Physical Therapy (DPT) degree or Master's degree from an accredited physical therapy program
  • Current and unrestricted New York State Physical Therapy License (required)
  • CPR/BLS Certification (required or must obtain upon hire)
  • Previous experience in skilled nursing facility (SNF), long-term care (LTC), or geriatric rehabilitation settings strongly preferred
  • Knowledge of Medicare guidelines and documentation requirements preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills with a patient-focused approach
  • Empathy, compassion, and collaborative team spirit
  • Proficiency with electronic medical records (EMR) systems preferred

Compensation & Benefits

  • Competitive annual salary: $104,000 - $124,000 based on experience and credentials
