Position Overview

We are seeking a dedicated and compassionate Physical Therapist to provide home-based rehabilitation services to adult patients throughout Franklin County, including the Tupper Lake and Malone areas. This is a direct hire position offering Monday through Friday daytime hours, allowing you to maintain a healthy work-life balance while delivering exceptional patient care in the comfort of patients' homes.

Key Responsibilities

  • Conduct comprehensive physical therapy evaluations using diagnostic muscle, nerve, joint, and functional ability tests and measures
  • Develop individualized treatment plans and provide evidence-based physical therapy interventions following physician orders
  • Perform initial patient assessments and periodic re-evaluations to monitor progress and adjust treatment plans accordingly
  • Create written problem lists and plans of care, including appropriate treatment types and frequencies
  • Document clinical notes on the date of service and maintain accurate, up-to-date medical records
  • Treat patients to address impairments and functional limitations with the goal of achieving maximum functional independence
  • Educate patients and caregivers on proper use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.)
  • Provide comprehensive patient and family education regarding treatment plans, goals, and home exercise programs
  • Communicate regularly with physicians and recommend modifications to plans of care as needed
  • Participate in physical therapy staff meetings, case conferences, and in-service presentations
  • Collaborate in the development and periodic revision of physician-ordered treatment plans
  • Ensure appropriate safety practices and proper maintenance of rehabilitation equipment
  • Travel to patients' homes throughout Franklin County to deliver high-quality physical therapy services

Required Qualifications

  • Graduate of an accredited Physical Therapy program (DPT or MPT degree)
  • Current, valid Physical Therapy license issued by New York State Department of Education
  • Minimum of one year of practical experience as a licensed physical therapist
  • Valid driver's license and reliable transportation for home visits throughout Franklin County
  • Strong clinical assessment and treatment planning skills
  • Excellent communication and interpersonal abilities to work effectively with patients, families, and healthcare teams
  • Proficiency in documentation and electronic medical records systems
  • Ability to work independently while managing a home visit caseload
  • Physical capability to perform medium work, including lifting up to 50 pounds occasionally

Preferred Qualifications

  • Experience in home health care or geriatric physical therapy
  • Certification in specialized treatment techniques or specialty areas
  • Knowledge of Medicare and Medicaid home health regulations
