Licensed Physical Therapist - Home Health | Full-Time & Part-Time | Saranac Lake, NY

Key Responsibilities

  • Conduct comprehensive physical therapy evaluations and assessments for adult patients in their homes following physician orders
  • Perform diagnostic muscle, nerve, joint, and functional ability tests and measures to determine patient status
  • Develop individualized treatment plans and establish goals to achieve maximum functional independence
  • Implement therapeutic interventions to address impairments, functional limitations, and mobility challenges
  • Educate patients and caregivers on proper use of assistive devices including walkers, canes, wheelchairs, and durable medical equipment
  • Document clinical notes, evaluations, progress reports, and treatment outcomes in electronic medical records
  • Perform periodic re-evaluations and modify treatment plans as needed in collaboration with physicians
  • Train and educate patients and family members on home exercise programs and therapeutic goals
  • Participate in interdisciplinary case conferences, team meetings, and continuing education opportunities
  • Maintain proper safety practices and equipment maintenance standards
  • Collaborate with physicians to develop and revise Plans of Treatment

Required Qualifications

  • Graduate of an accredited Physical Therapy program (DPT or MPT degree)
  • Current, valid New York State Physical Therapy license issued by the NYS Department of Education
  • Minimum one year of practical experience as a licensed physical therapist
  • Valid driver's license and reliable transportation for home visits
  • Strong clinical assessment and documentation skills
  • Excellent communication and patient education abilities
  • Ability to work independently in home care settings
  • Physical ability to perform medium work, including lifting up to 50 pounds occasionally
  • Capability to stand, walk, sit, stoop, kneel, and perform repetitive motions throughout the workday
