Position Overview

As a Home Health Physical Therapist, you will provide comprehensive rehabilitation services to patients in their home environments, helping them regain mobility, independence, and improved quality of life. Key Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments in patients' homes
  • Develop individualized treatment plans based on patient goals, diagnoses, and functional limitations
  • Deliver evidence-based therapeutic interventions including manual therapy, therapeutic exercise, gait training, and mobility training
  • Monitor and document patient progress using electronic medical records (EMR) systems
  • Modify care plans as needed based on patient response to treatment and changing conditions
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare team members
  • Educate patients and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques
  • Ensure compliance with Medicare guidelines, state regulations, and OASIS documentation requirements
  • Maintain professional standards of practice and participate in quality improvement initiatives
  • Manage caseload efficiently while maintaining flexible scheduling to meet patient needs

Required Qualifications

  • Current Physical Therapist license in the state of Texas (or ability to obtain)
  • Graduation from an accredited Physical Therapy program (DPT, MPT, or BS in Physical Therapy)
  • Current CPR/BLS certification from an approved provider
  • Valid driver's license and reliable transportation for home visits
  • Strong clinical assessment and critical thinking skills
  • Excellent verbal and written communication abilities
  • Proficiency with electronic health records and documentation systems
  • Ability to work independently with minimal supervision
  • Strong organizational skills and attention to detail
  • Physical ability to perform job duties including lifting, bending, and assisting patients with mobility

Preferred Qualifications

  • Previous experience in home health, skilled nursing, or geriatric physical therapy settings
  • Knowledge of Medicare home health regulations and OASIS documentation
  • Experience with wound care, orthopedic rehabilitation, or neurological conditions
  • Bilingual skills (English/Spanish) are a plus
