We seek a Senior Geriatric Care Manager for a leading Aging Life Care/ Care Management company. Based in Northern, VA, the Senior Care Manager must be an individual with a strong work ethic who is adaptable, dependable, and accountable. They will come with a solid background in care management and a ‘Rolodex’ of existing resources in Northern, VA. Please bring your deep passion for working with older adults to join our client, a private Care Management practice that has been serving seniors in Virginia for nearly two decades.
As the Senior Care Manager you will:
- Have PASSION for working with elders and the disabled
- Work as a team member utilizing your expertise, as well as the expertise and support of the team to ensure our clients receive the support they need, feel safe, secure, and empowered
- Be committed to advocating for the best quality of life for clients (in-home or at the facility) as well as being dedicated to the field of aging
- Complete comprehensive assessments & develop individualized care plans for each client
- Coordinate, manage, and oversee client care
- Bring clients to appointments
- Communicate with those involved in client’s care
- Visit clients in their homes, assisted living, and skilled nursing communities
- Perform quarterly client file audits, daily charting Manage your full client caseload, averaging 30 client billable hours/week
- Ability to attend monthly professional development meetings
Who Will Be a Great Fit?
- A master’s degree in a related field (or a BA/BS with 3+ years of experience in Gerontology (AL, SNF, CCRC, HomeCare)
- Care Manager Certification
- Certified Dementia Probationer (CDP), ideal
- Experience in-home care and/or skilled nursing/assisted living communities (required)
- Must have a "safety first" attitude towards all of our clients and staff, including being routinely tested and vaccinated against COVID-19 as well as other infectious diseases
- Ability to work on-call (rotating)
- A “team first” attitude and collaborative approach
- Licensed OT, MSW, RN, MFT, MA Gerontology or another related field, highly desirable
- Knowledge of local eldercare resources
- Experience and expertise working with clients with cognitive impairments (Alzheimer’s, Dementia, Parkinson's, etc.)
- Valid Virginia driver’s license, insurance, car
- Proof of being fully vaccinated
