The purpose of positions in this classification is to serve as a resource to referral sources, patients, and others to identify patient service and equipment needs, to initiate coordination of care, to initiate the admission and to smooth the transition to home care or hospice to meet patient needs.

Job Responsibilities: This listing is not to be construed as all-inclusive. Other duties may be required and assigned. • Answers phone calls to take patient referrals. Advises callers of company services and admission requirements and procedures. Communicates with referral sources and physicians to request the necessary information. Ensures for proper follow-up with referral sources as determined necessary. Reviews intake information specific to the patient referral to determine the appropriateness for home care or hospice. • Verifies insurance coverage. Contacts insurance providers to collect data on coverage and benefits. Completes insurance paperwork and processes as appropriate. • Serves as a liaison between the referral sources, patients, family members of patients, payers, physicians, healthcare personnel, home medical equipment providers and company staff to identify patient service needs and equipment needs, to initiate coordination of care, to initiate the admission, and to smooth the transition to home care or hospice to meet patient needs. Provides regular updates on the status of patients being admitted, on-hold, or re-admitted. • Enters patient demographic information, notes, and orders into the clinical database. • Coordinates with the Patient Care Managers on admissions so the proper information is in the clinical database system and information from the referral sources is forwarded to the appropriate office. • Coordinates with company Billing staff as necessary (e.g. Medicare cases, special insurance situations, etc.) • Completes all necessary documentation. Completes intake forms. Keeps accurate records of referral and admit statistics as required. Ensures for accuracy, completeness and timeliness of documentation. • Refers callers to other community resource providers as appropriate.
