Physical Therapist

Hornell, New York

We're seeking a dedicated Physical Therapist to join our team in Hornell, NY. In this role, you'll have the opportunity to make a significant impact on residents' lives in a skilled nursing setting while enjoying the beauty of New York's Finger Lakes region.

Key Responsibilities

• Conduct comprehensive evaluations of residents' conditions upon admission
• Develop and implement personalized physical rehabilitation plans
• Utilize various techniques including exercise therapy, manual therapy, and physical therapy devices
• Improve residents' mobility and pain management
• Educate residents on proper use of physical therapy equipment
• Monitor and document residents' progress throughout their recovery
• Collaborate with interdisciplinary teams to ensure holistic resident care
• Supervise physical therapy assistants as needed
• Maintain a safe, clean, and confidential working environment
• Adhere to infection control policies and all legal/professional requirements

Qualifications

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Experience in skilled nursing or long-term care (preferred but not required)
• Strong communication and interpersonal skills
• Commitment to resident-centered care and continuous professional development
