Senior Nurse Practitioner - Long-Term Care/Skilled Nursing Facility (LTC/SNF)
Join a leading healthcare organization as a Nurse Practitioner serving two premier skilled nursing facilities in the Glasgow, Kentucky area. Position Overview
As a vital member of our medical team, you'll provide primary care services to residents in long-term care and skilled nursing facilities, working independently while collaborating with an experienced healthcare team. This role offers a competitive salary range of $100,000-$150,000 with comprehensive benefits.
Essential Responsibilities
- Provide primary, urgent, and preventative care for SNF residents
- Conduct comprehensive resident assessments, physical examinations, and medication reviews
- Lead care planning and coordination with the interdisciplinary team
- Manage complex medical conditions and emergency situations
- Communicate effectively with families regarding treatment plans and status changes
- Collaborate with facility medical directors and attending physicians
Required Qualifications
- Current Kentucky Nurse Practitioner License
- Active DEA License
- National NP Certification
- Clinical background in adult, family, or geriatric care
- COVID-19 vaccination (unless qualified for exemption)
Preferred Qualifications
- LTC/SNF experience
- Background in internal medicine, gerontology, or cardiology
- Strong organizational and communication skills
- Demonstrated ability to work independently
