Senior Nurse Practitioner - Long-Term Care/Skilled Nursing Facility

Position Overview

We're seeking an experienced Nurse Practitioner to provide comprehensive primary care services in a skilled nursing facility setting. This role combines clinical excellence with a patient-centered approach, offering both professional autonomy and collaborative team support.

Key Responsibilities

  • Deliver primary, urgent, and preventative care to residents in assigned skilled nursing facilities
  • Conduct comprehensive initial assessments, physical examinations, and medication reviews
  • Manage complex medical conditions and coordinate care with interdisciplinary teams
  • Provide clinical leadership and collaborate with facility medical directors and staff
  • Communicate effectively with families regarding treatment plans and status changes
  • Ensure compliance with regulatory requirements and quality standards

Required Qualifications

  • Current Kentucky Nurse Practitioner License
  • Active DEA License
  • National Nurse Practitioner Certification
  • Clinical background in adult, family, or geriatric care
  • COVID-19 vaccination (exemptions available for qualifying circumstances)

Preferred Qualifications

  • Experience in geriatrics, internal medicine, or long-term care
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and organizational abilities
  • Demonstrated leadership and autonomous practice capability
  • Electronic Health Record (EHR) proficiency

Compensation & Benefits

  • Competitive salary range: $100,000 - $150,000
