Key Responsibilities
- Conduct regular patient assessments and develop comprehensive care plans
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
- Perform routine medical procedures and manage chronic conditions
- Order and interpret diagnostic tests and laboratory results
- Prescribe medications and monitor treatment effectiveness
- Document patient care using electronic health records (EHR)
- Participate in family conferences and care planning meetings
- Provide emergency care when needed
Required Qualifications
- Current New York State Physician Assistant License
- Board Certification as a Physician Assistant
- Graduate degree from an accredited PA program
- Experience in geriatric care or long-term care (preferred)
- Strong communication and interpersonal skills

Compensation & Benefits
- Salary Range: $115,000-$135,000
