Key Responsibilities

• Patient Care Management: Conduct comprehensive patient assessments and implement individualized care plans.
• Case Coordination: Serve as the primary liaison between patients, families, and interdisciplinary team members, ensuring continuity of care.
• Nursing Assessments: Regularly assess patients' physical, emotional, and psychological well-being.
• Care Planning: Develop, update, and monitor care plans tailored to each patient’s health needs.
• Patient Education: Empower patients and families by providing education on treatment plans, disease management, medications, and procedures.
• Documentation: Maintain detailed, accurate, and timely records, including assessments, care plans, and progress updates.
• Collaboration: Partner with physicians, therapists, and other healthcare team members to optimize patient outcomes.
• Supervisory Role: Provide oversight and guidance to home health aides and support staff.
• Compliance: Ensure adherence to all regulatory standards, policies, and procedures.
• On-Call Support: Participate in on-call rotation and provide weekend support as needed.

Qualifications

• Active Registered Nurse (RN) licensure in the state of Florida.
• At least 1 year of clinical experience in home health, nursing, or a related healthcare setting (preferred).
• Strong understanding of home health regulations, infection control practices, and documentation standards.
• Proven ability to develop and implement individualized patient care plans.
• Excellent interpersonal and communication skills, with a dedication to providing exceptional patient care.
• Current BLS certification (ACLS or additional certifications are a plus).
• Valid driver’s license and reliable transportation for field-based work.
