Clinical Pharmacist - Hospital and Ambulatory Care

We're seeking a dedicated Clinical Pharmacist to join our dynamic healthcare team in delivering exceptional pharmaceutical care. In this role, you'll collaborate with healthcare providers across multiple specialties, implement evidence-based treatment protocols, and directly impact patient outcomes through comprehensive pharmaceutical services.

Key Responsibilities:

• Provide expert clinical pharmacy services within interdisciplinary care teams
• Conduct medication therapy management and pharmacotherapeutic consultations
• Deliver virtual pharmacy consultations for ambulatory care offices
• Participate in emergency response for cardiopulmonary arrests
• Review and verify medication orders for safety and appropriateness
• Oversee sterile and non-sterile medication compounding
• Provide comprehensive patient and family medication education
• Mentor pharmacy students and residents through clinical rotations
• Maintain compliance with pharmacy regulations and institutional policies

Required Qualifications:

• PharmD degree from an ACPE-accredited College of Pharmacy
• Virginia State Board of Pharmacy license (or ability to obtain within 90 days)
• Strong clinical knowledge and decision-making abilities
• Excellent communication and interpersonal skills

Preferred Qualifications:

• Completion of PGY1 Pharmacy Practice Residency
• Hospital pharmacy experience
• Experience with electronic health record systems
• Background in ambulatory care services
