Senior Physical Therapist - Home Health Care

We're seeking a licensed Physical Therapist to join our patient-centered home health care team. In this role, you'll deliver exceptional therapeutic care while working independently with patients in their homes, collaborating with healthcare professionals, and making a meaningful impact on patients' quality of life.

Essential Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve patient mobility, function, and independence
• Create and implement home exercise programs tailored to each patient's needs and goals
• Educate patients and caregivers on therapeutic techniques, exercise protocols, and proper use of assistive devices
• Document patient progress and maintain detailed clinical records
• Collaborate with physicians and healthcare team members to ensure optimal patient outcomes
• Manage and prioritize patient caseload effectively
• Participate in care coordination and team meetings

Required Qualifications:

• Current Physical Therapy license in good standing
• Bachelor's degree in Physical Therapy (DPT preferred)
• BCLS certification (or ability to obtain within 30 days of hire)
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities
• Valid driver's license and reliable transportation
• Experience in home health care preferred
