Registered Nurse (RN) - Patient Care Coordinator

Essential Responsibilities:

  • Conduct comprehensive age-specific patient assessments and develop individualized care plans
  • Lead interdisciplinary care coordination and collaboration with healthcare team members
  • Implement and evaluate evidence-based nursing interventions to ensure optimal patient outcomes
  • Utilize EPIC electronic medical records system for accurate documentation and care planning
  • Administer medications and treatments following established protocols and safety guidelines
  • Serve as patient advocate while maintaining communication with families and care team members
  • Monitor and evaluate patient progress, adjusting care plans as needed

Required Qualifications:

  • Graduate from an accredited school of professional nursing
  • Current Virginia RN license or eligibility to obtain Virginia licensure
  • Current BLS/CPR certification (ACLS, PALS, NRP as required)
  • Proficiency with electronic medical records systems (EPIC)
  • Strong clinical assessment and critical thinking skills
  • Excellent interpersonal and communication abilities
