Clinical Program Manager - Home Health Services

We're seeking an experienced Clinical Program Manager to lead our home health services team. In this role, you'll oversee patient care quality, manage clinical staff, and ensure regulatory compliance while driving operational excellence. Core Responsibilities:

• Direct and oversee comprehensive patient care services, including care coordination, referral management, and individualized care plan development
• Lead quality assurance initiatives through program reviews and clinical outcomes analysis
• Manage and develop clinical staff through performance coaching, feedback, and professional development
• Ensure compliance with Federal, State, payor, and CHAP regulations
• Monitor and enhance patient satisfaction through data analysis and improvement initiatives
• Participate in budget development and management to optimize resource utilization
• Investigate and resolve patient concerns while maintaining service excellence
• Implement quality improvement initiatives based on clinical outcomes data

Required Qualifications:

• Bachelor's Degree in Nursing or related healthcare field
• Current licensure as a Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT)
• 3+ years of clinical experience in nursing/patient care
• Current CPR certification
• Valid driver's license and auto insurance
• Strong understanding of healthcare regulations and compliance requirements

Preferred Qualifications:

• Master's Degree in Healthcare Administration, Nursing, or related field
• 2+ years of supervisory experience in healthcare setting
• Home health care experience
• Experience with quality improvement methodologies
