We are seeking a highly skilled Clinical Pharmacy Specialist to join our prestigious transplant program. Position Overview

As a Transplant Clinical Pharmacy Specialist, you will play a crucial role in managing complex medication regimens for transplant patients, from evaluation through long-term follow-up care. This position combines clinical expertise, patient education, and research opportunities in a dynamic healthcare environment.

Key Responsibilities

  • Provide comprehensive medication management throughout the transplant continuum of care
  • Participate in multidisciplinary selection committee meetings and care planning
  • Conduct pre-transplant medication evaluations and patient counseling
  • Manage immunosuppressive therapy and complex medication regimens
  • Deliver extensive patient and family medication education
  • Monitor and adjust treatment protocols for rejection management
  • Precept pharmacy students and residents
  • Lead clinical research and quality improvement initiatives
  • Participate in protocol development and program advancement

Required Qualifications

  • PharmD from an ACPE-accredited College of Pharmacy
  • Completion of ASHP-accredited PGY1 Pharmacy Residency
  • Current Virginia pharmacist license or eligibility for licensure
  • Strong clinical decision-making and analytical skills
  • Excellence in interprofessional communication and collaboration
  • Demonstrated ability to work independently and manage time effectively

Preferred Qualifications

  • PGY2 Solid Organ Transplant Pharmacy Residency
  • Clinical experience in transplant pharmacy
  • Research experience and publication history
  • Teaching and precepting experience
