Position Overview
As a Transplant Clinical Pharmacy Specialist, you will play a crucial role in managing complex medication regimens for transplant patients, from evaluation through long-term follow-up care. This position combines clinical expertise, patient education, and research opportunities in a dynamic healthcare environment.
Key Responsibilities
- Provide comprehensive medication management throughout the transplant continuum of care
- Participate in multidisciplinary selection committee meetings and care planning
- Conduct pre-transplant medication evaluations and patient counseling
- Manage immunosuppressive therapy and complex medication regimens
- Deliver extensive patient and family medication education
- Monitor and adjust treatment protocols for rejection management
- Precept pharmacy students and residents
- Lead clinical research and quality improvement initiatives
- Participate in protocol development and program advancement
Required Qualifications
- PharmD from an ACPE-accredited College of Pharmacy
- Completion of ASHP-accredited PGY1 Pharmacy Residency
- Current Virginia pharmacist license or eligibility for licensure
- Strong clinical decision-making and analytical skills
- Excellence in interprofessional communication and collaboration
- Demonstrated ability to work independently and manage time effectively

- PGY2 Solid Organ Transplant Pharmacy Residency
- Clinical experience in transplant pharmacy
- Research experience and publication history
- Teaching and precepting experience
